Overview: create a template, so that every new document that users create will be a copy of the template. Great for forms.
Launch the full IE browser (not Edge)
- Navigate to the SharePoint site

Documents >(Click bottom-left link "Return to classic SharePoint")
- Tab "LIBRARY"

Advanced Settings
- Click the link "Edit template"
- Edit/save document
- When a user goes to that site, and navigates to Documents > New > Document, the new Word document will be a copy of that template

If you have a E3 or E5 license, then you'll qualify for using https://forms.office.com - and we recommend using the Excel Survey option to create rich forms, with automatic data collection, that can be shared with anyone, including users outside of Office 365 such as customers/clients.
